What to Bring

Here are a few tips to ensure your appointment runs smoothly:

Arrive at least 15 minutes early for your appointment. This extra time allows us to process your paper work prior to your seeing our medical staff.

Dress in attire that is conducive to a physical exam of your affected area. Appropriate clothing allows our staff to examine you with a minimal amount of disrobing on your part. For example, please wear/bring shorts for a knee condition or a tank top for a shoulder condition. Our office will gladly provide you with proper attire if needed.

Please bring all of the following to your appointment:

  • Driver's license or state issued identification card
  • Completed registration and medical history forms. To download these forms, click here.
  • Medical insurance card(s) including referrals from your family physician if required by your insurance company.
  • A list of questions and/or concerns for the doctor and his staff to answer.
  • Your method of payment; we accept cash, checks, Visa and Mastercard.
  • An up-to-date list of your current medications.

Cancellations

Lastly, if you are unable to keep your appointment please call us within 24 hours so we may use that reserved time for another patient.
