What to do I need to bring to my visit?
You will need to bring your drivers license and insurance cards for registration.
Bring appropriate clothing for you visit (ex. shorts for a knee injury or tank-top for a shoulder injury). We have clothing available if needed.
For a shorter wait, please fill out our registration packet, print it out and bring it in.

How do I cancel / reschedule my appointment?
If you need to cancel or reschedule an appointment please call our office as soon as possible so we may utilize that time for others. Call our reception at (517)263-0575 to let us know.
Where is your office located?
We are located at 227 Riverside Avenue, Adrian, MI 49221. Our satellite office in Dundee, MI is located at 100 Powell Drive, Suite 5. For directions to our offices click here.
